Navigating the Kentucky BioTech Facility

The mission begins with Case and Marshall entering a Kentucky BioTech facility filled with toxic gas, necessitating gas masks. An elevator malfunction leads to a fall and a gas mask breach, triggering hallucinations. After the ensuing cutscene, you regain control.

Locate the Security Desk and Initial Objectives

Explore the area until you find a locked red-lit door. Use a hatchet found on a mannequin to force it open. Proceed through the hallway, ascend the stairs in the main hall, and reach the central elevator. Activating the elevator transforms mannequins into zombies; eliminate them with your hatchet. A ringing phone at the circular desk directs you to the biotechnology room, requiring four director's cards (Red, Green, Blue, Yellow). You'll receive a map to the Yellow card.

Securing the Yellow Card and Grappling Hook

From the security console, follow the map to a yellow staircase. Solve the computer puzzle in the Director's office (answers: "Access" and "Lift"). In the A.C.R. room, more zombies appear. A mannequin holding the Yellow card transforms into an abomination upon approach. Before engaging, gather armor plates, weapons, and the crucial grappling hook. Use tactical explosives (C4 or grenades) to efficiently eliminate the abomination and its zombie horde. Retrieve the Yellow card.

Acquiring the Green Card

Use the grappling hook to return to the main facility. Locate the Administration Facility by grappling up from the security desk. Answer the ringing phone; the objective is to find and place four documents in the file display area. Mannequins will pursue you; maintain distance by sprinting. Document locations: a corner desk, near a round table, on a small central table, and in the cafe near a sink. After placing the documents, a red mannequin attacks; subdue it repeatedly with the grappling hook until it transforms into a Mangler Zombie. Obtain the Green card.

Obtaining the Blue Card

Grapple across from the Administration balcony to the Joint Projects Facility. Answer the ringing phone. Locate camera stands around a glass chamber containing the Blue card. A mimic will spawn; lure it out by shooting moving objects to eliminate it and collect the Blue card.

Retrieving the Red Card

From the security desk, go to the East Wing, following red carpets to stairs. In the resulting room, interact with the console to reveal the Red card, guarded by a Mangler. Use the grappling hook to reach the upper area, jump into the pool, and swim through a red tunnel. Climb ladders, eliminate zombies, and unlock a door using a blacklight-revealed code. Activate a timer and turn three drain switches within 25 seconds (one in the initial room, one in the unlocked room, and one accessible via grappling hook). After draining the water, follow the Mangler's path, defeat it and its horde, and retrieve the Red card.

Confronting the Disciple

Return to the security desk, insert all four cards, and take the elevator to the top floor. Eliminate any pursuing zombies. Answer the red phone, triggering a cinematic leading to a confrontation with numerous zombies and a Disciple in the BioTech room. Defeating them culminates in a final cinematic revealing the entire encounter as a hallucination.
